Medical Case Management

It can be challenging and time consuming to comprehend and coordinate complex medical care. Silver Oak medical case management supports families by attending appointments, advocating for the patient, facilitating communication across providers, and clarifying care plans for optimum outcomes.

Family Support

As needs increase, families may find themselves focused on providing care. We transition families from care providers so they can focus on meaningful time with loved ones.

Susan Wright

Susan brings a rare and deeply valuable blend of experience to Silver Oak — uniting her roots in clinical social work with over a decade of strategic consulting for mission-driven organizations. She holds a Master of Science in Social Work from Columbia University, where she specialized in aging and end-of-life care.

Susan began her career as a case manager supporting families navigating complex medical and social needs. Her frontline work with older adults and their caregivers grounded her in the practical and emotional realities of aging, serious illness, and family decision-making. Later, she transitioned into the world of technology and systems consulting, working with nonprofits and socially conscious enterprises to improve service delivery and operational impact.

At Silver Oak, Susan brings together her compassion, clinical expertise, and analytical rigor to provide bespoke, white-glove care management. She partners closely with older adults and their families to create clarity, calm, and dignity during life’s most vulnerable transitions.
