How It Works

The process begins with a complimentary 30 minute phone call to discuss and provide background information.  After the initial phone call,  a meeting will be scheduled and through meaningful conversation, Susan will gain a better understanding of the situation and priorities.  Also during this first meeting, the client and family members will sign releases for Susan to communicate with family and providers, sign a service contract agreement, and provide initial payment.
 
After the first meeting, Susan will document and share initial goals in order of priority. She will commence with services, remaining in close communication with the client and family.

Throughout this process, Susan will be available to attend appointments and provide ongoing support with the client and family as requested.
